The Tuck-and-Pull Technique

The tuck-and-pull technique is a modern surgical approach designed for the controlled, safe and effective explantation of posterior chamber phakic intraocular lenses (PCPIOL). The technique prioritises the protection of intraocular tissues and aims to minimise the risk of complications.

During the procedure, an ophthalmic viscoelastic device introduced through the side port increases the distance between the phakic lens and the crystalline lens. Because the existing surgical incisions can be reused in most cases, enlarging the main incision is not required. The haptic portion of the lens is grasped in a controlled manner with dedicated surgical instruments and removed in a single manoeuvre.

The tuck-and-pull technique provides a high level of protection for the corneal endothelium, the crystalline lens and the anterior chamber structures. It has established itself in the literature as a practical, effective and safe method applicable to all types of posterior chamber phakic intraocular lens requiring explantation for any reason.
