Visual, Refractive and Topographic Outcomes of Progressive-Thickness Intrastromal Corneal Ring Segments in Keratoconic Eyes

This study aims to evaluate the visual, refractive and corneal topographic effects of progressive-thickness intrastromal corneal ring segments (ICRS) implanted in patients diagnosed with keratoconus. Conducted as a multicentre, retrospective study, it analyses the one-year follow-up results of 58 eyes of 53 patients.

Following the procedure, significant improvement was found in patients' uncorrected and corrected visual acuity, a marked reduction in spherical and cylindrical refractive errors, and a decrease in maximum keratometry values. Significant improvements were also observed in the topographic indices reflecting corneal irregularity, and corneal stability was shown to be maintained throughout the one-year follow-up.

Progressive-thickness ICRS target the asymmetric and irregular structure of the cornea, providing a more controlled flattening effect. Thanks to these properties they deliver effective results particularly in asymmetric, oval or pellucid-like keratoconus types. No serious intraoperative or postoperative complications were reported in the study.

In conclusion, progressive-thickness intrastromal corneal ring segments stand out as a safe, effective and predictable surgical option in the treatment of keratoconus when applied with appropriate patient selection.
